It’s a rather long PDF in both Swedish and poorly translated English, with one form available in both Swedish and English that has to be filled in and sent back to the court.

To facilitate the process of filling it in (because the court thought it would be a good idea to force people to print it out and scan physical paper, not even making the PDF digitally fillable), we’ve created this page that can generate the filled in PDF form, where you can input your details here, and send the generated file to the court.
